My telehealth services are provided to you via secure and encrypted technology, which meets the standards for privacy and confidentiality required by law. I follow all state and federal laws and regulations regarding the practice of telehealth, and all medical services provided via telehealth are subject to the same standards of care as in-person medical care.
It is important to note that telehealth services may have certain limitations, including the inability to perform certain parts of a physical examination, which may result in difficulty making a precise diagnosis or providing treatment. Additionally, certain medications, for example, controlled substances, may not be prescribed through telehealth services.

To report a problem
If you believe your privacy rights have been violated, you may file a complaint with South Shore Wellness Family Health Nurse Practitioner, PLLC, or the Secretary of the Department of Health and Human Services, Office of Civil Rights, HIPAA, 200 Independence Avenue, S.W., Washington, DC 20201. We are bound by law not to retaliate should you file a complaint.
FEES AND PAYMENTS:
The practice DOES NOT file for insurance reimbursement. All services are paid for by the patient at the time of service. You may pay electronically, debit card, credit card.
Please know that we are here to help you if you have any questions. We are happy to help you learn how to file your own claim using the superbill we provide.
All outstanding balances must be paid in full prior to the next office visit or receiving supplements.
MISSED APPOINTMENT POLICY
We understand that life can be unpredictable, and occasionally, appointments may need to be rescheduled. However, we kindly ask that you notify us at least 12 hours in advance if you are unable to make your scheduled appointment.
Please note: No refunds will be issued for missed appointments.
If you need to reschedule, we will be happy to assist in finding a new time, but the missed appointment fee will still apply.
Thank you for your understanding and cooperation.
PHONE CALLS - Phone calls requiring 30 minutes or more of the provider's time will be charged as a minimum visit ($40/15 minutes).
COPIES - The cost for copies of chart notes, and invoices will be 50 cents per page, EXCEPT if requested at the time of the visit. Chart notes, and invoices pertinent to the visit will be provided free of charge on the day of the visit. Most documents will also be available for you on the patient portal.
SPECIAL LETTERS, FORMS, and DOCUMENTS - Completing workplace documentation, writing letters of medical necessity, etc. require significant provider time and will be charged an administrative fee of $10 per document/letter. Fees must be paid in advance. Some documentation may require extensive time / complexity and may justify a higher fee. If so, this fee will be disclosed to you prior to preparing the documents.
